Note: A minimum of 30 (thirty) days' notice is required for submitting a request for officials. Any submissions received with less than 30 days' notice may not be covered. Submitting the form does not guarantee that the GLOA will accept the games. You will be notified as soon as possible if there are any problems providing officials for the requested event.
