Important: This form must be submitted within 12 hours of the contest!

This information is used internally to prepare game reports that are submitted to either the GHSA or the NGLL. The form is only required if you have a qualifying event. Below is an excerpt from the GHSA web site defining the circumstances and reporting requirements. The GHSA form will be submitted based on the information that you provide.

The crew leader on the game is expected to file the report. Please be as complete as possible. Be aware of the required information prior to your games so that you know what information to gather at the game.

Ensure you enter the correct security code at the bottom of the form (this is used to prevent automated submissions.

Note:  The GHSA and NGLL rule for Expulsions and Disqualifications require the following:

  • In accordance with by-law 2.72, lacrosse players who are ejected from a game will also have to sit out the next game at their level of ejection and all other games in between.
  • Boys: any player ejected for accumulation of personal fouls will sit out one (1) game.
  • Any player or coach ejected for unsportmanslike behavior or violent conduct will sit out two (2) games.

GHSA Requirements

Game Reports from officials should be filed with the GHSA office as soon as possible for the following situations: (Note: Reports must be filed on the "Game Report" form as provided to the association secretary.)

  1. Ejections - Be certain to cite the name, jersey number and school of the ejected player or the name and school of the ejected coach and specify the behavior involved in a brief, professional manner.
  2. All protest situations - be certain to indicate the time at which the formal protest was lodged and the rule application that was protested.
  3. Any pre-game or post-game problems involving players, coaches, and/or fans.
  4. Any problems related to field/court conditions.
  5. Failure of host management to provide adequate security for officials at the game site.
  6. Any major injuries requiring extraordinary attention.
  7. NOTE: It is not necessary to report sportsmanship violations that do not result in ejections.

    The Game Reports should be factual narratives without any editorial comments. The comments should be written professionally so that they thoroughly describe the behaviors involved without any editorial comments. These reports are sent to the Principal of the school involved along with a letter from the Executive Director of the GHSA. One copy of this report should be sent to the GHSA Office, and a second copy should be kept with the association's records.

    Game Reports dealing with unusual situations such as protests, "intentional attempts to injure" or "multi-player fights" must be received by the GHSA within 24 hours of the end of that contest. The GHSA fax machine is operative 24 hours a day, as is the voice mail of the Executive Director.
